Below is a news release on a clinical report published in the July issue of Pediatrics, the peer-reviewed, scientific journal of the American Academy of Pediatrics (AAP).
For Release: July 7, 2003, 12:01 am (ET)
There has been an increase in the last several years in the number of children with flattened heads, according to a clinical report from the American Academy of Pediatrics (AAP) entitled, "Prevention and Management of Positional Skull Deformities in Infants." The report says the increase is probably due to recommendations by the AAP that children be put to sleep on their backs to prevent Sudden Infant Death Syndrome (SIDS). The report notes that the condition is, on the whole, preventable and treatable.
The report offers guidelines for the prevention, diagnosis and management of positional skull deformity in an otherwise normal infant. It stresses that pediatricians can advise parents as well as other health care professionals, such as those in newborn care units, on methods to decrease the risk of development of skull deformity.
A certain amount of prone positioning - or "tummy time" - while the infant is awake and being observed is highly recommended. Beginning at birth, most skull deformities can also be prevented by nightly alternating the head position while the baby is on his or her back - one night to the left, the next night to the right. In addition, parents can periodically change the orientation of the infant to the outside activity, such as is likely to occur at the door of their room. The report says infants should spend minimal time in car seats (when not a passenger in a vehicle) as this seating maintains the same head position as back sleeping.
The report examines the necessity of skull-molding helmets, and suggests that they are an option for patients with severe deformity or skull shape that is resistant to therapeutic physical adjustments and position changes.
According to the report, surgery is rarely necessary, but may be indicated in severe cases. Referral to a pediatric neurosurgeon, a general neurosurgeon with expertise in pediatrics, or a craniofacial surgeon should be considered if there is progression or lack of improvement following caregiver intervention.
EDITOR'S NOTE: This clinical report was published in the peer-reviewed, scientific journal of the American Academy of Pediatrics, but does not necessarily reflect the policies or opinions of the Academy. The American Academy of Pediatrics is an organization of 57,000 primary care pediatricians, pediatric medical subspecialists and pediatric surgical specialists dedicated to the health, safety and well-being of infants, children, adolescents and young adults.
